A second base

The long way round, explained honestly.

Phuket is a long way from the United States, and we say so first. We found no nonstop flight. The practical route is one change; nonstop flights reach Phuket from Seoul, Hong Kong, Taipei and Doha, and the fastest Los Angeles itinerary we found, via Hong Kong, takes about 20 h 25 min; other itineraries run to about 29 hours. That suits a stay measured in weeks or months rather than a long weekend, which is why the visa section below matters.

The practical questions come in a fixed order: what a month costs, where you would be treated, which visa fits, what to file. Medicare usually does not cover care outside the United States, so private cover is part of any plan. On the island, Bangkok Hospital Phuket is the name to start from; complex cases go to Bangkok, about 1.5 hours away by air.

The paperwork is shorter than people fear and stricter than they hope. Since 15 September 2026, visa-free entry for U.S. passports is 30 days; longer stays use the DTV, the LTR or a retirement visa. Per the IRS comparison page, a condominium you own directly is not reportable on FBAR or Form 8938, though foreign bank accounts above the thresholds are. After 180 days a year in Thailand you are a Thai tax resident, and Thailand taxes foreign income brought into the country; the LTR visa exempts it.

A monthly budget you can read

One agency’s April 2026 guide puts a comfortable month in Phuket at US$1,800–3,000, a one-bedroom rental at US$700–1,500, and December to March 30–50% dearer. They are estimates, not quotes, and the baht moved between 30.9 and 33.8 to the dollar in 2026. For a unit we give the common-area fee, the sinking fund and the taxes in writing.

A hospital with a name

Bangkok Hospital Phuket is described by an April 2026 expat guide as a full-service hospital with more than 300 beds and English-speaking specialists; complex cases go to Bangkok, about 1.5 hours by air. Medicare usually does not cover care abroad, so private cover is part of the plan. Ask any hospital for its accreditation and a written estimate.

The visa ladder, with its thresholds

Visa-free entry for U.S. passports is 30 days since 15 September 2026. The DTV asks for 500,000 baht in savings and gives five years, 180 days per entry. The LTR Wealthy Pensioner visa: age 50 or over, US$80,000 a year of passive income, or US$40,000–80,000 with US$250,000 in Thai assets. The O-A retirement visa: age 50 or over, 800,000 baht in a Thai bank. As of October 2026.

Freehold in your name, and one line for the IRS

A condominium unit is freehold within a building’s 49% foreign quota; foreigners cannot own land, and villas sit on a registered lease of up to 30 years. According to the IRS comparison page, a foreign home you own directly is not reportable on FBAR or Form 8938; foreign financial accounts are, above their thresholds.

Looked after while you are in the States

In a branded residence the hotel operator can rent, host and maintain the unit and report to you. Income depends on occupancy and is not guaranteed unless the contract says so. Rent is reported on your U.S. return and on the Thai one; we send you to a cross-border adviser for the arithmetic.

How long is the flight from Los Angeles, San Francisco, New York or Seattle, and where do I change?

We found no nonstop flight from the United States to Phuket. Nonstop flights reach Phuket from Seoul (Korean Air and Asiana, two a day), Hong Kong (Cathay Pacific and HK Express, three a day), Taipei (Tigerair Taiwan, twice a week) and Doha (Qatar Airways, two a day), according to a timetable aggregator on 10 October 2026. Gulf services have changed this year, so confirm Doha on the live schedule.

Seoul to Phuket is 6 h 10 on Korean Air (5 h 55 from 25 October) and 6 h 20 on Asiana. The fastest Los Angeles itinerary we found, via Hong Kong, takes about 20 h 25; others run to about 29 hours. The first leg from San Francisco, New York or Seattle depends on the carrier and the day, and we do not quote it — check the live timetable.

How long can I stay without a visa, and what are my options for longer?

Since 15 September 2026, U.S. passport holders may enter visa-free for 30 days, for tourism, with a possible extension of up to 30 more days at an officer’s discretion; land-border entries are limited to two a year. Older guides that say 60 days are out of date.

For longer: the DTV (age 20 or over, 500,000 baht in savings, a 10,000 baht fee, five years, 180 days per entry plus one 180-day extension; no work for Thai clients); the LTR Wealthy Pensioner visa (age 50 or over, US$80,000 a year of passive income, or US$40,000–80,000 with US$250,000 in Thai assets, US$50,000 of health cover, ten years, a 50,000 baht fee); the O-A retirement visa (age 50 or over, 800,000 baht in a Thai bank or an income test, and insurance of 400,000 baht inpatient and 40,000 baht outpatient — confirm with the embassy). Owning a condominium does not by itself grant any of them. Officers decide, and rules change.

Can I, as an American, own a condo in Thailand, and is it freehold?

Yes. A foreigner can hold a condominium unit freehold, in their own name, within the building’s 49% foreign quota; the title deed is issued in your name at the Land Office. Before a reservation we ask the building’s juristic person to confirm the quota in writing. The residences on this page start at $150,000.

Foreigners cannot own land. Villas sit on a registered lease of up to 30 years or on a Thai company, and Thai authorities pursued nominee companies in 2026, so a lawyer should read any structure first. Funds for a freehold condominium come from abroad in foreign currency, with the bank’s Foreign Exchange Transaction form.

Does Medicare cover me in Thailand, and what do I do about insurance?

Medicare usually does not cover health care outside the United States, and Part D does not cover drugs bought abroad, according to Medicare.gov. Plan on private international cover or a local policy, and ask the insurer whether it names the hospital you would use.

A retirement visa has its own insurance requirement: for the O-A, 400,000 baht inpatient and 40,000 baht outpatient, according to agent guides; confirm with the embassy. We are not insurers and do not quote policy prices.

What hospitals are on the island and how do costs compare?

We do not publish a price comparison, because we found no independent study. Ask any hospital for its accreditation certificate and a written estimate, and ask your insurer what it will pay.

Do I report a Thai condo to the IRS, and what are the FBAR and Form 8938 limits?

According to the IRS comparison of Form 8938 and FBAR, a foreign home you own directly is not a reportable asset on either form; if you hold it through a foreign entity, the entity is. Foreign financial accounts are reportable. The FBAR applies when your foreign accounts together exceed US$10,000 at any time in the year, and is filed with FinCEN. Form 8938, for a single taxpayer who qualifies as living abroad, applies above US$200,000 at year end or US$300,000 at any time, and for joint filers above US$400,000 and US$600,000; filers living in the United States have lower thresholds, so confirm which applies to you.

Rent from the condominium is reported on both your U.S. and your Thai returns. A Thai bank account you pay from counts toward the FBAR. We are not tax advisers; take these thresholds to a cross-border preparer.

Will Thailand tax my pension, Social Security or remote income?

A person who spends 180 days or more in Thailand in a year is a Thai tax resident. Since 1 January 2024, a resident is taxed on foreign income brought into Thailand, whenever it was earned. A relief for income remitted within two years was proposed in 2025 and, in the March 2026 guides we read, had not been enacted. The Long-Term Resident visa exempts foreign-sourced income.

Tax-preparer guides say Thailand does not tax Social Security under the U.S.–Thailand treaty and that there is no totalization agreement; confirm both with a cross-border adviser. The DTV does not allow work for Thai clients, and 180 days in the country makes you a resident for tax. Whether a private pension counts as foreign income brought into Thailand depends on how and when it is remitted; take it to a cross-border adviser.

What does a month actually cost, and what does the dollar rate do to it?

One agency’s April 2026 guide puts a comfortable month in Phuket at US$1,800–3,000 and a one-bedroom rental at US$700–1,500, with December to March 30–50% more expensive. Those are estimates for renting, not quotes. An owner’s month is the common-area fee, utilities and insurance; for a specific project we give the fee and the sinking fund in writing.

Costs are in baht. In 2026 the rate moved from 30.9 to 33.8 baht to the dollar, so the dollar value of the same baht bill varied by about 9%. Decide what range you can live with, and treat any dollar quote as a snapshot.

Is it safe, and who helps Americans in an emergency?

We do not rate countries. The State Department publishes the Thailand travel advisory at travel.state.gov, with its current level and date; read it before you plan, because the department updates it and we do not.

For consular help, the U.S. Embassy in Bangkok publishes its emergency contacts at th.usembassy.gov; check them before you travel. For health emergencies the island has Bangkok Hospital Phuket. In a hotel-branded residence the operator’s team looks after the building; ask for its contact details in writing when you reserve.

Where do the children or grandchildren go to school, and is there an American community?

UWC Thailand, in Phuket, teaches the International Baccalaureate from nursery to Grade 12, with about 500 students from more than 60 nationalities, according to Wikipedia. We have not verified other schools and will not list them here; tell us the ages and the length of stay, and we check the options on the call.

We found no survey of the American community on the island. What the sources show is that Americans are among the five largest groups of foreign buyers in Phuket (Krungsri Research, 28 August 2026) and appear among buyers in Bang Tao and Cherng Talay (CBRE, via Nation Thailand, 19 August 2026).
